Practical prompting and verification habits that keep AI output grounded in facts.
Hallucinations spike when a model has to guess. Feed it the source material instead: paste the document, the data, or the reference text into the prompt and tell it to answer only from what you provided. Adding "If the answer isn't in the text, say you don't know" stops the model from inventing a confident-sounding filler.
Be specific and constrain scope. Vague prompts like "tell me about X" invite drift; a narrow ask ("list the three pricing tiers from this page, with exact numbers") gives fewer gaps to fill. Ask for citations, quotes, or line references so every claim is traceable and easy to spot-check.
